Phase Two - Proposal

If the Daughters of Charity Foundation of St. Louis (DCFSL) has read the Letter of Intent and asked for your organization to submit a proposal, the following information must be in the proposal. Unsolicited proposals cannot be accepted.
 

Cover Page

  • Official name, address, telephone number, email and organization director/title of the requesting organization that has 501 (c) 3 status and can administer grant funds
  • Federal IRS tax ID number for the organization
  • Name, title and contact information of primary contact person for the request
  • Name of sponsoring organization. If independent, stand-alone organization, so indicate.
  • Project name & request amount
  • A statement signed by the Executive Director as follows: “I certify that the information provided in this grant request is complete and accurate based on currently available information. Any misrepresentations may be cause for revoking any grant monies to be, or previously, provided.”

Introduction
Describe your organization’s mission and vision. Provide a brief overview of organization programming, including the date the organization was founded.

Statement of Need
Explain the problem/need that prompted you to develop your project. Describe the population and geographic area served and the extent to which clients served are economically poor.

Project Plan Description
Describe your project. How will your work address the problem/need? What are the benefits of your project? Summarize project staffing, management, implementation/project time frame.

Objectives
This is a mandatory format for objectives. Provide at least five objectives that clearly describe what will be accomplished with your project. Each objective must describe what change or effect in behavior/knowledge/situation will occur for the client, to whom it will happen, to how many it will happen, what is being done to accomplish this change/effect and when the impact is expected to occur.

A sample objective is:
“Reduce the safety risk (change in situation) of 10 seniors (who and how many) by installing safety bars in 10 showers (what is being done) within 12 months of the grant award (when it will happen).”

Or
“Increase by 85% the parental understanding of appropriate discipline methods (change in knowledge) in 10 parents (who and how many) by educating them through five seminars (what is being done) within three months of the grant award (when it will happen).”

Evaluation
Describe the methods used to determine the project’s success.

Request
Indicate the specific amount requested and describe how the funds will be used. Comment on how this project will be supported or become self sustaining in the future.
 

Attachments:

  • A detailed project budget for your request
  • A listing of other funders being approached for your project including dollar amount request and current status of request
  • A copy of your organization’s
    • Current year budget
    • Balance sheets for the last two years*
    • Income statement for the last two years*
  • Federal/IRS tax exempt letter including tax ID number/proof of nonprofit 501(c) 3 status. If Official Catholic Directory (OCD) used for proof of nonprofit status, include copy of OCD page with your organization highlighted.
  • A signed statement of support for the project from your organization’s board chair
  • A listing of the organization’s Board of Directors
*If audited financial statements are not available, please indicate and furnish your internal financial reports.
