ALS Association St. Louis Regional Chapter Case Management and Respite Care for Older Adults with ALS $20,000 To provide free case management and respite services to older adults with ALS. Support and resources are also provided to their caregivers and family members to promote living independently as long as possible, cope with ever-changing symptoms, reduce emotional and financial stress and manage the progression of a fatal disease.
Arch City Defenders Inc. Veteran's Legal Services Project $30,000 To provide free, holistic legal services to veterans who are experiencing homelessness or at risk of homelessness. Providing these legal services helps remove barriers preventing veterans from achieving goals of employment, housing security, financial stability, and education.
Beyond Housing, Inc. (formerly Beyond Housing Neighborhood Housing Services) 24:1 Older Adult Engagement and Support Project $39,300 To develop a new project aimed at helping older adults access resources that facilitate living independently and support their engagement in the community. Agency will launch in-depth needs assessment to determine specific community needs including health care, housing modifications and accommodations, food security, and social interactions. 
Christian Friends of New Americans Christian Friends of New Americans $15,000 To provide support services for refugee and immigrant families that enhance their physical, educational, and spiritual needs. Services include English as a Second Language courses, health and wellness screenings, driver's education and fellowship activities led by ethnic leaders who speak the languages of the clients. 
Covenant Place Foundation Supportive Services for Older Adult Immigrant Chinese Residents $27,000 To provide culturally competent staff with language fluency to support Chinese older adults living at Covenant Place apartments with needed assistance in accessing available government benefits, community resources, and supportive services and other available services.
Eye Care Charity of Mid-America Mobile Vision Clinic: Hispanic Student Expansion Project $50,000 To provide free eye exams and prescription glasses to low-income Hispanic children in schools and community centers ensuring access to quality care for low-income immigrants and expanding services into additional Hispanic communities in Illinois.
Family Forward Multilingual Access Project for Immigrant and Refugee Survivors of Domestic Violence $35,000 To provide support services to non-English immigrant/refugee survivors of intimate partner violence. Services will include one-on-one language advocacy services including assistance completing various forms and paperwork, translation services, and action plans to address barriers in achieving economic security. 
Five Star Senior Center, Inc. General Operations $50,000 To provide a transportation program assisting older adults in St. Louis City to remain independent in their own homes by offering affordable, reliable, and accessible transportation to medical appointments, recreational activities, and necessary errands.
HOPE Housing Options Provided for the Elderly, Inc. HOPE Case Management $40,000 To provide case management services to connect older adults with benefits and services needed to promote wellbeing and increase older adults and their caregivers’ understanding of housing options that may enhance their safety and independence.
International Institute of Metropolitan Saint Louis Ensuring Quality Services for Refugees and Immigrants $49,447 To ensure quality provision of basic services for newly arrived refugees and foreign-born  St. Louisans. Funds will be used to support native language assistance and provide sufficient staffing to conduct core case management, mental health, employment, interpretation/translation, and economic development services. 
ITN St Charles  Transportation for Low-Income Older Adults in St. Charles and St. Louis County $31,838 To provide transportation services to older adults residing in St. Charles and St. Louis counties. Program provides dignified door-through-door transportation service  regardless of trip purpose, frequency, or destination.
Migrant and Immigrant Community Action  General Operations $50,000 To help immigrants overcome barriers to justice by providing legal services, organizing advocacy and education to promote the voice and human dignity of immigrant communities.
Northside Youth and Senior Service Center, Inc. Northside Older Adult Transportation $35,000 To provide city-wide door to door transportation services to older adults primarily in North St. Louis City. Project aims to preserve independence and mobility for low-income North City older adults by offering reliable and safe transportation.
Nurses for Newborns Foundation Immigrants Health Outreach Program $35,000 To meet the basic needs for infants in the St. Louis immigrant and refugee communities by employing mothers from these communities as partners in order to provide linguistically and culturally competent outreach, linking families with basic services including food, a safe living environment, and healthcare.
Places for People  Improving Outcomes for Older Adults with Serious Mental Illness $50,000 To support an ongoing project to reduce incidence of depression and isolation among older adults with serious mental illness. Program addresses physical and mental needs of low-income older adults through case management and recreational activities.
Rx Outreach, Inc. Health and Wellness Outreach $30,000 To provide healthcare workshops and follow-up with older adults residing in the St. Louis Metro area. Medical professionals provide evidence-based trainings, workshops and health screenings with a goal of reducing unnecessary hospitalization, and to remove racial and economic disparities by reducing incidence in low-income older adults. 
Saint Louis Inter-Faith Committee on Latin America (IFCLA) General Operations $36,000 Provide general operating support to the organization in supporting immigrants at mandatory immigration related check-ins. Service provides trained teams of community volunteers to accompany them and stand in solidarity during check-ins to reduce likelihood of detention and deportation while providing information and support to relatives in event of detention. 
Shepherd's Center of Webster-Kirkwood, Inc. Transportation Service Expansion $25,000 To support transportation program serving older adults residing in the Webster-Kirkwood areas of St. Louis. Service offers free rides to medical, dental and therapy appointments to independent, ambulatory older adults.
St. Andrew's At-Home Services, LLC  Project Independence $40,000 To keep older adults safe and in their own homes by addressing the increasing need for quality minor home repair for low-income older adults over the age of 65 who live in St. Louis City and County.
St. Francis Community Services Veterans Advocacy Project $50,000 To provide free legal education and aid to veterans who cannot afford an attorney through informational sessions and advocating for the veteran in the courts system and Veterans Administration.
Visiting Nurse Association of Greater St. Louis 52 Touchpoints Older Adult Loneliness Prevention Program $40,000 To launch a pilot program with aim to reduce loneliness among older adults and, in doing so, reduce the associated risks of negative health outcomes linked to loneliness. Program will link volunteers with homebound elders and provide weekly visits that will include socialization, assessment, and education components.
